Biosurgery (also known as maggot or larval therapy) is a promising addition to a wide range of topical treatment options, especially for debridement. The application of living maggots on wounds to remove devitalized tissue, reduce the risk of infection, and promote wound healing is referred to as “biosurgery.” Although Lucilia sericata larvae are essential in entomology, their propensity to feed on necrotic tissue and microbes makes them ideal candidates for chronic wound treatment. Chronic wounds, such as diabetic foot, have recently become major healthcare issues; their financial impact is too great, and chronic wound prevalence is on the rise. Maggots carry out these functions through mechanical movements (such as eating) and secretions/excretions that include a variety of proteins, enzymes, and chemicals. Furthermore, recent research has revealed that maggots respond differently to wounds and infectious agents, with proteins displaying a wide range of variability depending on the lesion and infectious agent encountered. This appears to be an ideal adaptation mechanism, allowing clinicians to deliver “patient-specific” treatment.
Biosurgery is a cost-effective, simple-to-use approach for treating chronic wounds. Several actions were documented in several investigations, despite the fact that the effect mechanisms are yet unknown. These activities are incompatible and should be assessed as a single-chained reaction circle.
